When a Form is created in a group, it automatically generates a designated folder path (Apps > Microsoft Forms > Form Name > Question Name) in the default SharePoint document library. If this folder is moved or deleted, the upload breaks. Manually recreating the folder does not restore the backend sync.

Access SharePoint Site Contents

Navigate to the private group's SharePoint site. Click the gear icon in the top right corner and select 'Site contents'.

Open the Recycle Bin

In the Site contents menu, locate and click on 'Recycle bin' in the upper right area or the left-hand navigation pane.
